Résumé

Pré-requis : Les manifestations thoraciques des lymphomes malins sont fréquentes et très variées. Elles sont habituellement d’apparition secondaire rarement révélatrices.
Buts : Etudier les caractéristiques cliniques, radiologiques et pronostiques des manifestations thoraciques révélatrices des lymphomes, en mettant en exergue la diversité de ces manifestations et les difficultés diagnostiques et thérapeutiques qui en découlent.
Méthodes : Nous rapportons une analyse rétrospective des atteintes thoraciques révélatrices de lymphomes ainsi que les différents aspects radiologiques observés chez 17 patients hospitalisés au service de pneumologie de Sfax durant 17 ans entre 1989 et 2005.
Résultats : Il s’agit de 10 cas de lymphome non Hodgkiniens (LNH) et de 7 cas de maladie de Hodgkin (MH). Chez les patients atteints de LNH, l’atteinte médiastinale était présente dans 8 cas. L’atteinte parenchymateuse était notée chez 2 patients, sous forme de nodules ou de masses associées à des adénopathies (ADP) médiastinales. L’atteinte pleurale sous forme de pleurésie ou de masse solide était présente dans 3 cas. Les atteintes péricardique et pariétale étaient notées dans un seul cas.Chez les patients atteints de MH, les ADP médiastinales étaient présentes chez 6 patients. L’atteinte parenchymateuse était notée dans un seul cas, à type de micronodules sous pleuraux. Un cas de lymphome primitif thymique et un cas de lymphome trachéal étaient notés. Aucun cas d’atteinte pleurale n’a été relevé.
Conclusion : Les manifestations thoraciques révélatrices des lymphomes, rarement décrites, sont caractérisées par leur diversité et leur manque de spécificité. Leur diagnostic, souvent délicat, a largement bénéficié des progrès de l’imagerie médicale et des explorations endoscopiques. En revanche, la prise en charge thérapeutique aurait besoin d’être optimisée, afin d’améliorer le pronostic qui demeure encore réservé notamment pour les LNH
